SWD "Community Care Service Voucher for the Elderly" Scheme

In 2020, we officially become one of the approved service provider under the "Community Care Service Voucher for the Elderly" scheme launched by the Social Welfare Department, with reference number RSP9501, and provide on-site home care and home support services to eligible seniors. Provide 100 home care service places, tailor-made various highly flexible service plan combinations for the elderly with different health conditions and needs, day, evening and night services are charged at a flat rate to actively meet the different needs of the elderly.

 


Professional Medical Team

The institution provides a wide range of professional nursing services and has a senior medical team including physiotherapist, occupational therapists, speech therapists, physiotherapy assistants, occupational therapist assistants, registered nurses, enrolled nurses, health workers, and nursing assistants.

Elders who hold "Community Care Service Vouchers for the Elderly" cover all 18 districts in Hong Kong. 

 

How can I use the community voucher to make an appointment for home service?

 

Step 1: Apply for Community Voucher

SWD will invite eligible elders or their carers to participate in the Community Voucher Certificate Scheme. For application details, please contact the social worker. Eligible elders include those who are on the Central Waiting List for Subsidized Long-Term Care Services and have been assessed as moderate to severe impairment under the SWD’s Unified Elderly Services Assessment Mechanism; and have not received any residential care service or subsidized community care service 's elders.

 

Step 2: Contact OK Care

After the elderly or their carers receive the certificate of service voucher, they can contact the OK Care case manager by calling 6922 0711 to follow up. The manager will first formulate a suitable personal care plan according to the needs of the elderly.

 

Step 3: Go through the formalities

After knowing the details of the service, the elder or his carer needs to sign the service consent form, and the manager will assign a professional nursing staff to come to the elderly having evaluation.

 

Step 4: Enjoy the service

Professional medical staff will come to provide services for the elderly.
